UNDERSTANDING YOUR SCREENING QUESTIONNAIRES

Mental health can feel complicated, but the tools we use to understand it don’t have to be. The screenings you complete during your visit help us get a clearer picture of how you’ve been feeling so we can support you in the best possible way.

Depression Screeners

  • PHQ-9 (Patient Health Questionnaire-9)
A short, 9-question form about your mood over the past two weeks.

    • BDI-II (Beck Depression Inventory)
      A more detailed 21-question form.
  • Helps identify psychological, emotional, and physical symptoms of depression.

Anxiety Screeners

  • GAD-7 (Generalized Anxiety Disorder-7)
A 7-question tool about worry, tension, and feeling on edge.
  • Helps us understand how anxiety shows up and how it interferes with your life.
    • BAI (Beck Anxiety Inventory)
A 21-question form that focuses on the physical symptoms of anxiety, like restlessness or tightness in the chest.

Substance Use Screeners

  • AUDIT (Alcohol Use Disorders Identification Test)

  • Helps identify patterns that might be risky or harmful.
    • DAST-10 (Drug Abuse Screening Test)

Why These Tools Matter
These questionnaires are:

  • Quick
  • Confidential
  • Non-judgmental
  • Helpful for treatment planning

They don’t diagnose you on their own. Instead, they give us a starting point for conversation and help us track your progress over time. Many clients find that seeing their scores change can be encouraging and motivating.
